At Evolve Therapy Collective, we are dedicated to fostering a supportive and inclusive environment, where our clients feel heard, valued, and empowered to lead fulfilling lives.

Sensory Processing

We offer occupational therapy sensory processing assessment and interventions to help individuals effectively interpret and respond to sensory stimuli, promoting improved regulation, engagement, and participation in daily activities.

The way in which our brain receives, organises and responds to the sensory information in our environment affects the way that we behave and is the foundation for many other life skills.  If a person misinterprets everyday sensory information, such as sound or touch, they may feel overwhelmed, may seek out sensory experiences or avoid certain sensations. This can cause strong emotional responses or behaviours which may affect their ability to participate in their activities of daily living, work or community.

Our Occupational Therapists can:

  • Assess concerns around sensory issues by conducting a detailed sensory profile using appropriate standardised assessment tools e.g. Adult/Adolescent Sensory Profile as well as clinical observation and interview with family/carers;
  • Recommend and prescribe interventions including accommodations and adaptations, environmental modifications, sensory equipment and sensory diets.
  • Provide education to the person and their family/carers about the influence of sensory functions on occupational performance and how to minimise their negative impact on function; and
  • Provide recommendations on how to proactively help prevent and de-escalate maladaptive behaviours.

These interventions are carefully tailored to help individuals improve sensory modulation, integration, and discrimination, thereby enhancing their ability to engage effectively in their daily routines, social interactions, and learning experiences. Whether it’s using sensory-based strategies, providing a sensory-rich environment, or recommending specific sensory tools and techniques, an OT’s expertise in sensory assessments and interventions makes a significant difference in promoting optimal sensory functioning and improving the overall quality of life.

Functional Capacity Evaluations

Our occupational therapists conduct comprehensive functional capacity evaluations, carefully assessing individuals’ abilities and challenges, and deliver detailed reports with recommendations to guide…

 informed decision-making on effective interventions.

A functional capacity evaluation (FCE) is a comprehensive assessment conducted by an occupational therapist to determine an individual’s physical, cognitive and emotional abilities related to work or daily activities. The main purpose of an FCE is to gather objective information data on a person’s functional capabilities, strengths, limitations, and potential for improvement so that the occupational therapist can make recommendations for appropriate accommodations, modifications and/or levels of support required. and inform return-to-work decisions.

Our clinicians are trained in a range of standardised assessments which are used in conjunction with clinical observation of a person engaging in their everyday activities and interviews with a person and their family, carers and/or other professionals/supports.  Here at Evolve Therapy Collective, we have a proven track record of producing highly comprehensive reports that support NDIS plan reviews, assisting NDIS participants to access tailored support and services that are precisely aligned with their unique needs, ensuring that they receive the appropriate interventions, therapies, and accommodations to maximise their independence, improve their quality of life, and actively engage in their chosen activities and pursuits.  Working collaboratively with a person-centred approach is at the heart of our practice, ensuring that individual needs and preferences guide every aspect of our interventions and care.

Our occupational therapists provide their expertise across a diverse age spectrum, spanning from teenagers to the elderly population, and are experienced in a range of areas including complex mental health conditions, autism spectrum disorder, young onset dementia and neurological conditions.

Assistive Technology

Clinicians skilled in the assessment and prescription of assistive technology are able to provide innovative solutions that promote independence and improve the quality of life for the individuals we work with.

Our clinicians are passionate about supporting clients to overcome barriers and challenges that impede their engagement in everyday activities, social and community participation, work and leisure interests.  One size does not fit all, and our occupational therapists are skilled in assessing individuals to address their unique needs, ensuring that the identified assistive technology will suit that individual and their lifestyle and promote independence.

Assistive technology can include items such as mobility aids, communication devices, adaptive tools for daily living, environmental control systems, computer and mobile accessibility software, sensory aids and wearable technology.  Our clinicians will collaborate closely with clients in the home, educational environment, or workplace, to evaluate their specific needs and challenges.

Through personalised trials and hands-on guidance, our occupational therapists will facilitate a comprehensive exploration of the chosen technology, ensuring its optimal functionality and empowering clients to confidently embrace its benefits in their daily activities.

Wheelchair Prescription

Our experienced occupational therapists use their in-depth knowledge of mobility and seating principles to assess individual needs and recommend appropriate wheelchair options that ensure optimal fit and…

function, to enhance mobility and independence.

The role of an occupational therapist in wheelchair prescription is paramount in enhancing mobility and independence for individuals with mobility challenges. At Evolve Therapy Collective, our experienced OTs are specialised in assessing the unique needs of each participant to ensure the most suitable wheelchair is prescribed. Through comprehensive evaluations, they consider factors like physical abilities, lifestyle, and personal goals to select the appropriate type, size, and features of the wheelchair. With their expertise in assistive technology and adaptive equipment, our OTs tailor the wheelchair to optimise comfort and functionality. Beyond just providing a mobility aid, they focus on empowering individuals to actively engage in their daily activities, fostering a sense of confidence and well-being. Our goal is to enable our clients to move freely, participate fully in their communities, and lead fulfilling lives with the utmost independence and dignity.

Minor and Major Home Modifications

We use our knowledge and practical skills to evaluate each individual’s unique needs in order to recommend appropriate modifications that facilitate the creation of accessible and safe living environments…

that promote independence and support a person’s functional needs.

Occupational therapists play a pivotal role in facilitating minor home modifications that cater to individuals’ unique needs and promote enhanced independence. By conducting thorough assessments of the living environment, our experienced occupational therapists identify areas where simple yet impactful adjustments can be made. Whether it’s installing grab bars, improving lighting, rearranging furniture for better accessibility, or ensuring proper pathways, these modifications are carefully tailored to accommodate physical limitations and create a safer, more accommodating living space. Through their expertise, our occupational therapists strive to empower individuals to navigate their homes with confidence, fostering a sense of security and enabling a greater degree of autonomy in daily activities.

Our experienced occupational therapists are also skilled at the assessment and design of complex home modifications.  Equipped with a deep understanding of both the functional requirements and the physical limitations of their clients, our occupational therapists will collaborate closely with builders, contractors and other professionals to design comprehensive solutions. These modifications may encompass structural changes, such as widening doorways for wheelchair accessibility, installing elevators or ramps, creating accessible bathrooms, and implementing specialised kitchen setups. Through their expertise, our occupational therapists not only enhance the physical accessibility of living spaces but also strive to foster an environment that promotes well-being, autonomy, and a higher quality of life for individuals facing complex mobility issues or disabilities.

Home and Living Assessments

Our team of occupational therapists are skilled in conducting Home and Living assessments, which encompass Supported Independent Living (SIL), Independent Living Options (ILO) and Specialist Disability Accommodation (SDA).

Our range of comprehensive assessments under the NDIS encompasses Supported Independent Living (SIL), Specialist Disability Accommodation (SDA), and Individual Living Options (ILO). These assessments are designed to establish a person’s baseline functional ability in relation to their everyday instrumental and personal activities of daily living and provide clear, objective recommendations regarding that person’s support needs.  Our occupational therapists will undertake detailed functional capacity evaluations, which will include standardised assessments, clinical observation, interviews with the participant, their families and carers as well as liaison with key stakeholders and professionals involved in the individual’s care.  With extensive experience in professional report writing, our occupational therapists ensure that the NDIS receives clear and comprehensive information to make well-informed SIL, ILO and SDA decisions based on the reasonable and necessary criteria. The structure of our reports facilitates the reader in establishing explicit connections between the participant’s abilities, support requirements, and the need for specialist housing, all aligned with NDIS requirements.

At Evolve Therapy Collective, we empower participants to achieve greater independence by finding more suitable and accommodating living arrangements. We are dedicated to ensuring that individuals with disabilities receive personalised assessments that cater to their unique needs and aspirations, and we will always work collaboratively with clients to ensure their needs, wants and preferences are heard and communicated. With our client-centred approach and expertise in NDIS assessments, we are here to support participants on their journey towards greater independence and improved quality of life.

Life Skills Training
